Looking for Flexibility and a Rewarding Career in the Field of Hospice?
AuthoraCare Collective is currently seeking a PRN CAP Case Manager (Social Worker) for our Burlington, Greensboro or High Point campus.This is a hybrid position.
Manage the over-all plan of care for a minimum case load of CAP clients as determined by the Manager of CAP programs. Assess and identify patient/caregiver problems, establish goals, and interventions. Monthly client contact and quarterly in-person visits for assessment of ongoing needs, interventions, and service utilization.
Requirements
Education/Qualifications:
  • Graduate of an accredited School of Social Work
  • One year of experience in social work case management required
  • Bachelor's degree in social work required
  • Two years of experience preferred
  • Case management accreditation or certification preferred by agency.
  • Other: Valid State-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Delivery of Client Service Coordination
• Directs and coordinates all services provided through CAP to patients in accordance with the patient's individualized care plan.
• Utilizes case management process in planning and prioritizing patient care needs, including assessment and identification of patient/caregiver problems; identification
and implementation of interventions; and evaluation of outcomes.
• Partners with the assigned RN case manager to complete annual recertification process.
• Establishes relationship with patient/caregiver to adequately assess ongoing needs.
• Recognizes influence of culture, ethics, and beliefs on decision making.
• Makes home visits in accordance with NC Medicaid requirements.
• Offer telephonic support to CAP clients as needed.
• Communicates changes to the plan of care to patient/family and any internal/external stakeholders involved.
• Complies with infection control and exposure control policies.
• Maintains effective relationships and partnerships with community healthcare providers and external stakeholders impacting the CAPC population.
