CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.
Job Summary
The Community Bridges Supportive Services for Veteran Families' (SSVF) Health Care Case Manager will act as a liaison between the SSVF Grantee and the VA or community medical clinic working with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans. The SSVF Health Care Case Manager will provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. The SSVF Health Care Case Manager will work closely with the Veteran's primary care provider and members of the Veteran's assigned interdisciplinary treatment team. The SSVF Health Care Case Manager will possess excellent judgment and have at least two years of experience in a health care or social services area of practice. The SSVF Health Care Case Manager works closely with the Veteran's assigned multidisciplinary team, including medical, nursing, and administrative specialists, and case management personnel. The SSVF Health Care Case Manager works collaboratively within this team to provide timely, appropriate, Veteran centered care equitably services to identify and address systems challenges for enhanced care coordination as needed. Sensitivity to all Veterans' individual needs concerning age, developmental requirements, and culturally related factors must be consistently achieved.
Skills/Requirements
• Highschool diploma or GED required. Associate degree in the field of social sciences preferred.
• Applicants must have experience working with individuals with complex needs, particularly in behavioral health or care coordination.
• Minimum of one (1) year of experience providing case management, coordinating community resources, and collaborating with interdisciplinary teams is required.
• Experience working with veterans or vulnerable populations is strongly preferred.
• Familiar and knowledgeable of available community resources, preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders preferred.
• Current AZ Driver's License (valid and in good standing).
• Clean 39-month Motor Vehicle Record (MVR).
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures required.
• Fingerprint Clearance Card required.
• Peer Certification (if applicable).
