Bridges Healthcare, Inc. has an immediate opening for a Care Coordinator, in their Family Centered Services Division in New Haven. (This position is Bilingual English/Spanish preferred, but not required).Salary: $24.80 - $30.00/per hour, commensurate of experience. Benefits for the positions include:Competitive pay commensurate with experience Medical and Dental with significant employer contribution to the premiums and HSA (if chosen as your plan)403(b) with a generous employer contributionEmployer-Paid Life InsurancePaid Vacation with 22 days to start, paid personal days, paid sick time, paid winter holiday break days, plus 11 paid holidays PURPOSE AND POSITION OVERVIEWThe Pediatric Care Coordinator provides culturally competent services that link children and youth with special health care needs and their families to services and resources in a coordinated effort to maximize the potential of the children and provide them with optimal health care.  RESPONSIBILITIES:Assist in identifying children with special health care needs at each medical homeConduct patient and family assessments as requested by families or providers to identify needs, strengths, and resourcesEngage children, youth, and families in the development of an individualized Care Plan based on family strengths, needs, and goalsCoordinate implementation of Care Plans, including ongoing monitoring and periodic evaluationMake referrals, work with providers to arrange enrollment or admission, and help families access desired or needed servicesMake home visits based on the needs of children, youth, and familiesProvide children, youth, and families with information, education, and support regarding medical conditions, available resources, and effective partnerships with healthcare providersAssist families to access needed respite care, extended services funds, and other resourcesCoordinate communication, referrals, and information sharing among agency programs, providers, and community agencies to reduce duplication of servicesMaintain collaborative relationships with program partners, specialists, healthcare providers, and community resourcesDevelop and maintain relationships with community and statewide resources and participate in related groups and organizations, including The Family Support NetworkConduct data collection activities as required by agency and funderAttend staff, intake evaluation and interagency meetings as requiredAttend and actively participate in regularly scheduled supervisory meetingsMaintain accurate records of all client contact and collateral workComplete all record keeping and evaluation activities as requiredAttend educational training in related areasPromote the agency's services, programs, image and philosophyMaintain timely and complete individual case files and strict confidentialityPerform any other department or agency-related duties or special projects as directed by supervisor.QUALIFICATIONS:Bachelor's degree in social work, nursing or other related field or Registered Nurse licensed in CT, BSN.Experience with children and youth who have special health care needs and their families preferred. Experience working as part of a multidisciplinary team. Demonstrated knowledge of treatment planning process. Strong problem-solving, written and verbal communication skills. Knowledge of community resources.
