Family Centered Services of CT, a division of Bridges Healthcare, Inc. has an immediate opening for a Care Coordinator.
(This position is Bilingual English/Spanish preferred, but not required).
Salary: $24.80 - $30.00/per hour, commensurate of experience.

PURPOSE AND POSITION OVERVIEW
The Pediatric Care Coordinator provides culturally competent services that link children and youth with special health care needs and their families to services and resources in a coordinated effort to maximize the potential of the children and provide them with optimal health care.
RESPONSIBILITIES:
  • Assist in identifying children with special health care needs at each medical home
  • Conduct patient and family assessments as requested by families or providers to identify needs, strengths, and resources
  • Engage children, youth, and families in the development of an individualized Care Plan based on family strengths, needs, and goals
  • Coordinate implementation of Care Plans, including ongoing monitoring and periodic evaluation
  • Make referrals, work with providers to arrange enrollment or admission, and help families access desired or needed services
  • Make home visits based on the needs of children, youth, and families
  • Provide children, youth, and families with information, education, and support regarding medical conditions, available resources, and effective partnerships with healthcare providers
  • Assist families to access needed respite care, extended services funds, and other resources
  • Coordinate communication, referrals, and information sharing among agency programs, providers, and community agencies to reduce duplication of services
  • Maintain collaborative relationships with program partners, specialists, healthcare providers, and community resources
  • Develop and maintain relationships with community and statewide resources and participate in related groups and organizations, including The Family Support Network
  • Conduct data collection activities as required by agency and funder
  • Attend staff, intake evaluation and interagency meetings as required
  • Attend and actively participate in regularly scheduled supervisory meetings
  • Maintain accurate records of all client contact and collateral work
  • Complete all record keeping and evaluation activities as required
  • Attend educational training in related areas
  • Promote the agency's services, programs, image and philosophy
  • Maintain timely and complete individual case files and strict confidentiality
  • Perform any other department or agency-related duties or special projects as directed by supervisor.

QUALIFICATIONS:
  • Bachelor's degree in social work, nursing or other related field or Registered Nurse licensed in CT, BSN.
  • Experience with children and youth who have special health care needs and their families preferred.
  • Experience working as part of a multidisciplinary team.
  • Demonstrated knowledge of treatment planning process. Strong problem-solving, written and verbal communication skills.
  • Knowledge of community resources.
  • Access to reliable transportation on a consistent basis, valid driver's license and auto insurance.
