Job description

Position Summary:
This individual will provide behavioral health support to residents of long term care facilities under the supervision of a licensed professional. They may receive training in a broad range of assessments and interventions that will support treatment specified in individual care plans. Regional travel will be required.
Essential Duties and Responsibilities:
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive, other duties may be assigned:
  • Conduct assessments that include the behavioral and social needs of the patients
  • Meet with families and facility staff to coordinate care as recommended by licensed professionals
  • Assess the patient's knowledge of their clinical condition and provide education and self-management support
  • Provide outreach to clients who are having problems with treatment adherence
  • Assist in the development of the individual goals and behavioral health treatment plan
  • Coordinate with other agencies and community providers
  • Maintain timely documentation of work
  • Serve as an active member of the behavioral health treatment team

Education and/or Certifications:
  • Bachelor's degree in Counseling, Social Work, Psychology or related field

Skills and Qualifications:
  • Experience in a psychiatric or long term care setting preferred
  • Customer service experience preferred
  • Ability and willingness to function independently and proactively in a long term care setting
  • Effective use of electronic medical records and Microsoft software (teams, excel spreadsheets, etc)
  • Skilled in written and verbal communication.
  • Skilled in exercising a high degree of initiative, judgment and discretion.
  • Ability to handle confidential and sensitive information.
