Location: Cleveland Ohio Area (within 30 miles)
Schedule Options: PRN, PT, morning, afternoon, evening shifts available
Pay Rate: $45-$52/hour

Job Responsibilities

Clinical Care & Patient Monitoring
• Perform comprehensive in-home patient assessments
• Monitor vital signs, symptoms, and overall condition
• Identify and escalate changes in patient status

Skilled Nursing Care
• Administer medications (oral, IV, injections)
• Perform wound care, IV therapy, catheter care, and other procedures
• Execute physician orders and individualized care plans

Care Coordination
• Collaborate with physicians and interdisciplinary teams
• Coordinate labs, diagnostics, and additional services
• Ensure seamless care transitions and continuity

Patient & Family Education
• Educate patients and caregivers on disease management and medications
• Provide guidance on recognizing warning signs and next steps

Documentation & Compliance
• Maintain accurate EMR documentation in real time
• Follow all regulatory, HIPAA, and company standards

Emergency Response
• Act quickly in urgent situations and initiate appropriate interventions
• Coordinate hospital transfers when necessary

• Follow strict infection prevention protocols
• Maintain a safe and clean care environment


Job Qualifications & Requirements

Education & Experience
• Active Registered Nurse (RN) license (Ohio)
• Minimum 1-2 years of RN experience (home health, ER, ICU, or med-surg preferred)
• Experience with IV therapy, wound care, or acute care strongly preferred

Requirements
• Local candidate within 30 miles of Cleveland area
• Valid driver's license, reliable transportation, and auto insurance
• Willingness to travel between patient homes

Compliance
• Current CPR certification
• Negative TB test (within 12 months)
• Up-to-date immunizations or exemptions (COVID, Flu, MMR, Hep B)
• Ability to pass background check and drug screening
