Registered Dietitian Position Summary:

Sweetwater Care is seeking a Registered Dietitian to join our team in a skilled nursing setting. This role offers flexibility to support a variety of schedules, including full-time, part-time, or hybrid opportunities depending on candidate preference and community needs.

Core Responsibilities:

· Complete timely, accurate nutrition assessments, follow-ups, and documentation in accordance with CMS regulations and facility policies

· Develop and implement individualized nutrition care plans that align with physician orders and resident goals

· Monitor residents at nutritional risk, including weight changes, wounds, tube feeding, and altered intake

· Collaborate with nursing, therapy, food service team, and medical providers to ensure coordinated care

· Ensure diet orders are accurate, implemented correctly, and communicated effectively to the food service team

· Support survey readiness through compliant documentation and participation in audits as needed

· Provide education to residents, families, and staff as appropriate

· Participate in Quality Assurance and Performance Improvement (QAPI) activities

Qualifications:

· Must be a Registered Dietitian with CDR (Commission on Dietetic Registration)

· Must hold a state license in the state of which you are applying if indicated

· 1 Year experience preferred
