Summit Home Care and Hospice is searching for an experienced and motivatedย Registeredย Nurse (RN)ย to join our Home Health team.ย At Summit, we believe healthcare should be personal, compassionate, and delivered where it matters mostย -ย at home.ย 

Registeredย Nurse:ย 

  • Hours:ย Monday Friday;ย Standard Business Hours & Occasional On-Call Rotationย 

  • Work Setting:ย On the Road for Patient Visits & Training at the Local Office or Remotelyย ย 

  • Travel:ย Required +ย Mileage Reimbursedย 

  • Annual Salary:ย $75,000 โ€“ $85,000 (Overtime Eligible)ย ย 

  • Location: Dublin, OH & Surrounding areasย 

Job Summary:ย ย 

The Registered Nurse (RN) plans, organizes and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.ย ย 

Responsibilities:ย ย 

Patient Careย 

  • Completeย an initialย assessment of patients and family toย determineย home care needs.ย ย 

  • Provide a complete physical assessment and history of current andย previousย illness(es).ย ย 

  • Regularly re-evaluate patient nursing needs.ย ย 

  • Initiate the plan of care and make necessary revisions as patient status and needs change.ย ย 

  • Use health assessment data toย determineย nursing diagnosis.ย ย 

  • Develop a care plan, whichย establishesย goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions; include the patient and the family in the planning process.ย ย 

  • Initiateย appropriate preventiveย and rehabilitative nursing procedures.ย ย ย 

  • Administer medications and treatmentsย as prescribed byย the physician.ย ย ย 

  • Provide direct patient care as defined in the State Nurse Practice Act.ย ย 

  • Counsel the patient and family in meeting nursing and related needs.ย ย 

  • Provide health care instructions to the patient as appropriateย perย the assessment and plan of care.ย ย 

  • Identifyย discharge planning needs as part of the care plan development and implement prior to discharge of the patient.ย ย 

  • Act as the Case Manager when assigned by Clinic Manager and assume responsibility to coordinate patient care for assigned caseload.ย ย 

Communicationย 

  • Prepare clinical notes and update the primary physician when necessary and at least everyย 60 days.ย ย 

  • Communicate with the physicianย regardingย the patient's needs and report any changes in the patient's condition; obtain/receive physician's orders asย required.ย ย 

  • Communicate with community health related people to coordinate the care plan.ย ย 

Additional Dutiesย 

  • Participate in on-call duties as needed and defined by the on-call policy.ย ย 

  • Ensure that arrangements for equipment and other necessary items and services are available.ย ย 

  • Instruct, supervise, and evaluate home health aide care provided every two (2) weeks.ย ย 

Qualifications:ย ย 

  • Associate or bachelorโ€™s degree in nursing from as accredited school of nursing.ย 

  • A valid/active RN license.ย ย ย 

  • Prior home health care or acute care experience preferred.ย ย ย 

  • Management experience preferred but not required.ย ย ย 

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.ย ย 

  • Requires prolonged or considerable walking or standing and considerable reaching, stooping, bending,ย kneelingย or crouching.ย ย 

  • Must be able to lift, position or transfer patients and lift supplies and equipment.ย ย ย 

  • Must have visual acuity and hearing to perform required nursing skills.ย
