What You'll Do

As a Home Health LPN, you will provide skilled nursing care under the direction of a Registered Nurse and physician while supporting patients and families throughout their healthcare journey.

Responsibilities include:

  • Delivering care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of Care

  • Monitoring patient conditions and reporting significant changes to the RN Case Manager and healthcare team

  • Assisting patients and caregivers with education and self-care techniques

  • Implementing patient safety plans and utilizing available community resources

  • Collaborating with the interdisciplinary care team to ensure exceptional patient outcomes

  • Documenting clinical findings, nursing interventions, and medication updates accurately and timely

  • Identifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits

  • Providing compassionate, professional support to patients, families, physicians, and colleagues

Qualifications

Required:

  • Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program

  • Current Massachusetts Licensed Practical Nurse (LPN) license

  • Minimum of one (1) year of clinical nursing experience

  • Valid driver's license, reliable transportation, and current auto insurance

  • Ability and willingness to travel throughout the assigned territory

  • Physical Requirements- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
