Are you a compassionate and detail-oriented LPN (Licensed Practical Nurse) looking to make a meaningful impact in home healthcare? Excel Home Care Services is seeking a dedicated LPN (Licensed Practical Nurse) to provide Medication Management to patients in their homes.
Position Overview:

As a Medication Management LPN (Licensed Practical Nurse), you will play a crucial role in ensuring the safe and effective administration of medications for our clients. You will conduct in-home visits, educate patients and caregivers, and collaborate with healthcare professionals to enhance patient well-being.
Key Responsibilities of LPN (Licensed Practical Nurse) :

  • Conduct in-home assessments to review and manage client medications.
  • Educate clients and caregivers on proper medication usage, potential side effects, and adherence.
  • Set up and organize medications, including pre-filling medication planners when necessary.
  • Collaborate with physicians, pharmacists, and the care team to address medication-related concerns.
  • Monitor clients for adverse reactions and report any significant health changes.
  • Maintain accurate documentation of medication administration and client progress.
  • Ensure compliance with healthcare regulations and company policies.
  • Conduct thorough health assessments, including checking vital signs and implementing care plans.
Qualifications:

  • Active LPN (Licensed Practical Nurse) license in the state.
