We're actively seeking a compassionate and skilled LPN to join our dedicated team. Here, you'll be an integral part of our residents' daily lives, helping them maintain their vitality and independence.


As an LPN, you'll work under the direction of a Registered Nurse (RN) to deliver efficient and effective nursing care, ensuring positive clinical outcomes and resident satisfaction.

Position Highlights
Collect, report, and document patient data, observe and report changes in condition to the RN.

Contribute to individualized patient care plans, including establishing goals and developing interventions.

Administer medications and perform treatments as ordered by physicians.

Communicate pertinent patient data to the RN and/or physician.

Monitor and supervise patient care provided by unlicensed staff, ensuring job duties are performed according to policies and procedures.

Utilize Point Click Care (PCC) for documentation and maintain accurate drug counts.

Qualifications:

Must be a graduate of an approved school for practical or vocational nursing.

Current Licensed Practical Nurse license by the State Board of Nursing in the center's state is required.

Must be able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients to respond to their medical and physical needs.

Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy.

Maintain current BLS/CPR certification (as required by State).
