Are you a visionary leader in home health and hospice ready to embrace innovation and improve patient identification and home services transitions? Dominican Hospital in Santa Cruz, CA is offering an exciting hospital-based role: Health at Home Advisor.

The Health at Home Advisor (HHAd) - BSW has expert knowledge of home services and is an integral member of the healthcare team.The HHAd- BSW educates selected, pre identified, at-risk populations about home-based services capabilities, preparing patients and families for support needed at home.  The HHAd - BSW collaborates with the hospital care team to identify and facilitate care for patients whose clinical outcomes and satisfaction would benefit from home services.  They function  as a resource for the patient, family, and hospital care team to ensure the patient receives timely and appropriate home services. May serve multiple hospital locations  within a management market.

Essential Key Job Responsibilities
  1. Supports CommonSpirit mission to make the healing presence of God known in our world by improving the health of the people we serve, especially those who are vulnerable, while we advance social justice for all. Consistently lives People First Behaviors.  Follows the policies and procedures of the organization.
  1. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information. 
  2. Works with hospital partners to identify and prioritize patient populations who will benefit from HHAd services. 
  3. Initiates post discharge care setting discussion with identified patients and families, initiating ‘why not home’ conversations and acting as a resource for the patient and the care team to realize home care alternatives. Guides patients through and around barriers within the healthcare system.
  4. Identifies opportunities to reduce both financial and clinical risks to patients and families  discharging from the hospital. 
  5. Acts as an active participant in multidisciplinary rounds as a patient advocate to promote appropriate level of care and to ensure efficient continuity of care throughout the continuum. 
  6. Collaborates with physicians and staff around care planning.
  7. Communicates pertinent care destination information and the home services candidates who were identified to the case manager and/or social worker. 
  8. Maintains communication with patients, families, and health care providers to monitor patient satisfaction.
  9. Collaborates with Care Coordination to facilitate follow up services with home services providers
  10. Maintains documentation of coordination of home Care/hospice services.
  11. All other duties as assigned.
Job Requirements
  • Completion of an accredited BSW program. Current unrestricted license as a BSW in state(s) of practice.
  • Home Health or Hospice experience within the last 10 years required. Prior navigator experience in a post-acute setting such as ALF/SNF/ILF preferred. 
  • Combination of Acute and Post-Acute care delivery experience preferred.
  • Three years social work experience in the health care setting preferred
  • Excellent computer skills and ability to learn new systems.
  • Strong organizational (time management) and  interpersonal skills.
  • Ability to handle multiple priorities with strong attention to detail. Knowledge of and practical use of good business spelling and arithmetic.
  • Ability to communicate effectively using written and verbal skills. Proficient in email communications and internet usage along with basic use of Microsoft Excel and Word. 
  • Knowledge of information technology to evaluate care effectiveness (care process, outcomes and cost). 
  • Ability to work autonomously within a matrix environment without direct supervision or support.
