Position Summary
Reporting to the Assisted Living Director and partnering closely with the Clinical Leader/Case Manager, the Resident Care Coordinator serves as a key leader within the neighborhood.
This role provides day-to-day support and guidance to Resident Care Aides and Medication Assistants while ensuring residents receive compassionate, high-quality care that meets Department of Health regulations and Glenmere standards. The successful candidate will balance clinical oversight, staff support, resident advocacy, and family communication while helping create a positive and enriching environment for both residents and team members.
This position offers a unique opportunity for an experienced LPN to share their knowledge, mentor newer caregivers, and play a meaningful leadership role in a close-knit assisted living community.
What You'll Do
- Provide leadership, guidance, and support to Resident Care Aides and Medication Assistants
- Help onboard, mentor, and train new team members
- Participate in interviewing, hiring, and staff development activities
- Assist with resident service plans and ensure care needs are met safely and effectively
- Partner with physicians, residents, families, and interdisciplinary team members to coordinate care
- Support compliance with Department of Health regulations and community policies
- Monitor resident well-being and proactively address changes in condition
- Assist with medication management oversight and follow-up
- Foster strong relationships with residents and families through regular communication and support
- Contribute to a positive, respectful, and resident-focused culture
- Serve as a trusted clinical resource for staff and families
- Participate in the on-call rotation and provide occasional support with direct care and medication administration when needed
Qualifications
- Current New York State Licensed Practical Nurse (LPN) license
- Prior assisted living, long-term care, senior living, or resident care experience preferred
- Previous supervisory, lead, charge nurse, or mentoring experience is a plus
- Strong communication and relationship-building skills
- Passion for serving older adults and supporting caregivers
- Ability to work independently while collaborating effectively with a team
- Commitment to resident dignity, safety, and quality of life
