Outreach Director (Part-Time)

Location: Woodbury, MN (Travel throughout the Twin Cities Metropolitan Area)
Pay: $26.00 per hour
Schedule: Part-Time, 30 hours per week

About A Place Called Home LLC

A Place Called Home LLC is a Minnesota 245D licensed Home and Community-Based Services (HCBS) provider committed to helping individuals with disabilities and behavioral health needs live safely and independently within their communities. We provide Individualized Home Supports (IHS), Individual Community Living Supports (ICLS), Homemaking, Night Supervision, and 24-Hour Emergency Assistance (24EA).
Our mission is to deliver compassionate, person-centered services while building meaningful partnerships throughout Minnesota that improve access to quality community supports.
Position Summary

This is a newly created position and will play a critical role in helping A Place Called Home achieve its strategic growth goals by expanding our 24-Hour Emergency Assistance (24EA), IHS, and ICLS referral network throughout Minnesota.
We are seeking an enthusiastic, self-motivated Outreach Director who enjoys networking, building professional relationships, and connecting individuals with life-changing community services. This position is ideal for someone with experience in healthcare, social services, behavioral health, community outreach, or case management.
This is an excellent opportunity for someone looking to grow with an expanding organization. Following a successful six-month performance review, employees who demonstrate strong performance, leadership, and measurable success in developing referral partnerships may be considered for additional hours, increased responsibilities, or promotion into a full-time leadership position as the organization continues to grow.
Primary Responsibilities

  • Develop and maintain relationships with county case managers, social workers, hospitals, treatment facilities, psychiatric hospitals, behavioral health providers, homeless shelters, sober living facilities, probation offices, parole offices, the Minnesota Department of Corrections, housing organizations, and other community partners.
  • Conduct in-person visits to referral sources throughout the Twin Cities metropolitan area.
  • Introduce A Place Called Home LLC and educate community partners on our services.
  • Identify and cultivate new referral opportunities.
  • Build long-term professional relationships that generate ongoing referrals.
  • Represent A Place Called Home at networking events, resource fairs, conferences, and community meetings.
  • Track outreach activities and referral sources using a CRM or referral tracking system.
  • Collaborate with leadership to develop outreach strategies and marketing initiatives.
  • Maintain regular communication and follow-up with referral partners.
  • Assist with presentations, marketing campaigns, and community education efforts.

Qualifications

Required

  • Bachelor’s degree in Human Services, Social Work, Healthcare Administration, Marketing, Business, or a related field (or equivalent experience).
  • Excellent verbal and written communication skills.
  • Strong relationship-building and networking abilities.
  • Highly organized with excellent time management skills.
  • Valid driver’s license, reliable transportation, and proof of automobile insurance.
  • Proficiency with Microsoft Office and Google Workspace.
Preferred

  • Experience working within Minnesota’s HCBS or 245D service system.
  • Experience in healthcare marketing, behavioral health, social services, or community outreach.
  • Existing relationships with hospitals, counties, treatment providers, or community organizations are highly desirable.
