This Discharge Planning Assistant opportunity is full time at 40 hours per week, offers full benefits and a convenient flexible schedule. We are looking for a dynamic individual who

supports the hospital team discharge planning functions for patients who are in an inpatient setting. Facilitates the continuum of care and through-put process of transitioning patients, to the appropriate level of care while ensuring the most cost effective and safe environment.

Job Duties/Responsibilities:

  • Assists the case managers and social workers with the communication and collaboration of the discharge care plan – which may include the patient interview, gathering pertinent clinical information, initiating referrals, i.e. home health, DME, arrange transportation.
  • Data entry of patient interview and planning documentation, level of care, claims data and other case related information.

  • Arranges with Nursing Staff appropriate times to transfer patients by Ambulance/Medair./MedExpress or through Logisticare for Medicaid patients.

  • Calls, manages, and uses Navihealth to send HHC the necessary referral paperwork under the direction of the Case Manager for anticipated discharge.
