Catholic Charities of Orange, Sullivan & Ulster, is looking for a full-time Case Manager - CHASSI in their Sullivan County Case Management program. 
The hourly rate for the position is $23.00/hour.  This position comes with benefits.
Summary:
This position provides direct case management services to individuals who are needy or 
disenfranchised in Sullivan County. Responsibilities include advocacy, information and referral 
services, crisis intervention, and short-term case management. The role also encompasses reception 
duties, such as answering telephones and processing clients for services. Additionally, the 
position will participate in community outreach events to connect individuals with prevention, case 
management, and community-based peer recovery services offered by our agency.
Essential Duties and Responsibilities include the following, Other duties may be assigned.
1. Conduct initial intakes utilizing the Apricot Platform database system.
2. Actively participate in system-wide data sharing through integrated digital referral systems 
(e.g., Unite Us) or other approved platforms.
3. Prioritize intakes and ensure warm handoffs throughout the CCOSU system and with community 
partners, ensuring all intake forms, screening tools, and notes are completed thoroughly and in a 
timely manner.
4. Attend Parent-Child Program training and deliver services effectively in accordance with program 
guidelines.
5. Participate as a member of an interdisciplinary treatment team to ensure all program
participants receive the full benefit of CCOSU resources.6. Maintain current, accurate, and complete documentation of all services provided to clients.
7. Answer telephones, provide information, and assist clients as needed.
8. Develop and maintain effective working relationships with service providers in Sullivan County.
9. Attend community and service provider meetings as required.
10. Participate in outreach activities during weekends and after-hours, including community tabling 
events, pop-up pantries, food pantry outreach, Stuff-The-Bus, and other case management activities.
11. Maintain an updated community resource list/guide to use as a referral source for clients.
12. Provide crisis intervention services to homeless individuals and families experiencing housing 
emergencies.
13. Serve as a primary case manager for emergency relief funds.
14. Conduct thorough needs assessments and develop short-term service plans to address identified needs.
15. Conduct social care screenings of Medicaid members using the standardized AHC Health-Related Social Needs Screening Tool in the Unite Us platform, referring cases to the Social Care Manager as appropriate.
16. Assist individuals who are not eligible for enhanced services in navigating and accessing 
local, state, and federal programs.
17. Log all client interactions thoroughly in Unite Us, Apricot, and other required systems.
18. Maintain accurate and complete case files, documentation, records, and other required materials.
19. Process cases within a reasonable timeframe, preferably within 45 days.
20. Assist weekly with food and children’s pantry distributions.
21. Participate in additional outreach and special projects, including but not limited to 
Stuff-The-Bus (June and November), Thanksgiving Basket (November), Adopt-A-Family and Giving Tree (Christmas), and Back-to-School supply distributions (August).
22. Participate in regular supervisory sessions with the immediate supervisor.
23. Attend required agency in-service trainings.
24. Travel to meet clients at other CCOSU offices as needed.
25. Provide coverage on other sites as needed.
26. Perform other duties as assigned by the supervisor or as required by program needs.

Position Type and Expected Hours of Work:
This is a full-time position. Standard work hours are generally Monday through Friday, 9:00 AM to 
5:00 PM. This position may require evening and weekend work when community events are
scheduled; these events are planned.  Additional hours may be required to meet program
deadlines or client needs.
Working conditions and physical demands required:
The demands described here are representative of those that must be met by an employee to 
successfully perform the essential functions of this job.
•  Ability to work with the public and/or under potentially stressful conditions.
•  Basic understanding of client engagement and de-escalation techniques.
•  Prepare comprehensive progress notes and service plans
•  Ability to lift/move at least 50 pounds.
•  Remain stationery at a workstation and use a computer at least 30% of the time.
Qualifications
Education and/or experience required:
•  Associates degree with one-year experience in Human Services required.
•  Bachelor’s degree in social work or related field, highly preferred.
•  Must possess and maintain a clean NYS Driver’s License throughout employment and have reliable 
transportation.
Skills, Licenses, and/or competencies required:
•  Knowledge of Social Services available in Sullivan County preferred.
•  Bilingual (English/Spanish). Preferred but not required.
•  Computer literate with knowledge of Microsoft Word, Excel, and Outlook.
•  Excellent interpersonal and communication skills.
•  Good organizational, time-management and communication skills.
•  Individual must possess the ability to work well independently as well as part of a 
team.
