We are currently seeking anย RN to be a part of ourย team. The ideal candidate will provide coverage for After-Hours to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
Schedule:
Thursday - Monday.; 4:00pm-12:30am.
Location:
Marin County with additional coverage to San Francisco & Sonoma Counties as needed
Essential Duties & Responsibilities includeย (but are not limited to):
- Make in-person nursing visits to patients as assigned, or as indicated.
- Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
- Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
- Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
- Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
Education/Experience:
- Associate's or Bachelor's degree in nursing from an accredited School of Nursing
- Experience in acute care/ home health and/or hospice setting preferred
Certificates, Licenses, Registrations:
- Current license as a registered nurse in the State of California.
- Public Health Nursing certification highly desirable.
- HPNA certification preferred.
Pay Range:
$55.50 - $69.50 + after hours/weekend shift differential
