Job Title: MDS Nurse Assessment Coordinator for AFC in Brockton
Location: Brockton, MA
Compensation: $100  per MDS Nurse Assessment and $50 per visit

About the Position
We are seeking a caring and detail-oriented Registered Nurse to join our AdultFoster Care (AFC) program as an Initial Assessment / Admission Nurse. In thisrole, you will complete in-home evaluations for individuals applying to the AFCprogram, reviewing their needs and confirming eligibility. This opportunity isideal for nurses with experience in geriatrics, home health, or community-basedcare who are committed to supporting individuals and enhancing quality of life.


Key Duties

  • Conduct comprehensive in-home assessments for new AFC applicants.
  • Evaluate medical, functional, and psychosocial conditions to determine eligibility.
  • Partner with caregivers, families, and healthcare professionals to design customized care plans.
  • Provide education to participants and families regarding AFC services, expectations, and program policies.
  • Ensure compliance with all state regulations and AFC program requirements.
  • Maintain accurate, up-to-date documentation of assessments, care plans, and follow-up activities.


Qualification

          Demonstratedability to coach, train and provide staff development

          Proficiencyin Microsoft (Word, Excel and PowerPoint)

          MS or Bachelor'sdegree in Nursing

          Must befully licensed RN by Mass Board of Registration

          At leastfive years of previous home health care experience
