Brockton Home Health Care AFC is looking for A registeredNurse (RN)/Case Manager to manage and support our Adult Family/Foster to makesure the our programs runs smoothly.
Jobdescription
• Responsible for overall supervision of all HomeVisits RN's;
• Completing the MDS-HC and other clinicalassessments;
• Developing and reviewing on an ongoing basiseach member's AFC/GAFC plan of care;
• Training, evaluating, and supervising AFC/GAFCcaregivers in conjunction with Program Director;
• Reviewing the physician summary;
• Monitoring the health status of all members toensure that all needed AFC/GAFC is properly delivered;
• Reporting changes in the health status of anymember to the member's physician;
• Periodically reviewing AFC/GAFC caregiver logs;
• Conducting admission on-site visits for newmember;
• Completing a nursing progress notecorresponding with each on-site visit or encounter;
• Submitting a semiannual health-status report tothe member's primary care physician;
• Planning for an implementing discharges fromthe AFC/GAFC program;
• Conducting an orientation for each AFC/GAFCcaregiver before the AFC caregiver begins personal care;
• Providing ongoing training to AFC/GAFCcaregivers on health and aging
• Any other office duties that may be assigned byProgram Directors.
Qualification
• Demonstratedability to coach, train and provide staff development
• Proficiencyin Microsoft (Word, Excel and PowerPoint)
• MS or Bachelor'sdegree in Nursing
• Must befully licensed RN by Mass Board of Registration
• At leastfive years of previous home health care experience
