Active Day Healthcare Coordinator

Daily pay available! Choose a career with meaning at Active Day where you make a difference!

Full-time, part-time, and per diem positions available.

What sets Active Day nurses apart from others? You care for our members without working 12-hour shifts, being on call, staying late daily, and working with increased patient ratios that change day-to-day and, at times, unsafely.

At Active Day, you provide "true care" to your members. You will "get to know" your members and provide the person-centered care you've always wanted to provide. You will rarely be required to handle acute care conditions day-to-day, and most centers will have aides to assist you as needed.

Unlike hospitals and residential care, you will have manageable duties. You will become familiar with your members' healthcare needs and have various support personnel that you can trust and rely on!

Job Summary

The healthcare coordinator provides coordination of care needs to members to improve health status and quality of life, delay/deterioration of health status, manage chronic conditions in collaboration with the physicians, and integrate medical and social services.

The healthcare coordinator ensures that all work complies with company policies and procedures, safety and regulatory laws and standards. The healthcare coordinator works closely with our members and their support team, which includes but is not limited to the care management team, family/caregiver, center director, IHCC director and nurse manager within the center, as well as the member's treating practitioners.

Qualifications
  • MSW or LPN required, LPN or RN preferred.
  • Valid malpractice insurance policy and registered with the applicable state examining board.
  • MSW must have a minimum of two (2) years of experience providing healthcare coordination.
  • Valid driver's license and proof of insurance/registration.
  • Experience working in a health care facility, preferably working with seniors.
  • Active CPR certification required.
  • Strong communication and engaging interpersonal skills.
  • Very strong organizational skills and ability to shift priorities quickly.
  • Able to work a flexible schedule.
  • Successful results of background check, and where applicable, additional state-specific regulatory requirements.
Job Responsibilities
  • Development and oversight of a healthcare support plan that includes coordination of medical care and proactive care management of both chronic diseases and complex conditions such as recurring falls, depression, and dementia.
  • Collaboration with treating practitioner(s) (i.e. physicians) on member care.
  • Member medication review.
  • Transitional care from hospital or nursing facility to home/assisted living.
  • Advanced care planning.
  • Can aid in the assessment and care planning for members in collaboration and under the supervision of the nurse manager, director of nursing, and/or supervising registered nurse.
  • Assist and/or completes required documentation for admission paperwork and members health record.
  • Provides health education and instruction to the member and their families as needed.
  • Ability to travel up to 25% in the local market.
  • Communicates in a positive, courteous, and professional manner with members, their families, support team, and community at large at all times.
  • Participates in staff meetings and in-service educational programs; actively collaborates with cross-functional team, providing feedback and recommendations for enhancing members' quality of care and life as a whole.
  • Complies with company policies and procedures, safety and regulatory law and standards.
  • Provides assistance to center team and members as needed (i.e. with proper certification, assists with passing out member medication).
  • Other duties as changed or assigned at any time.
