Job description

The Community Health Access Manager is responsible for overseeing all Patient Services functions, including clinical specialty referrals and the coordination of connections to community resources such as housing, food, and transportation. This role leads outreach efforts and assesses patient barriers to care and services, with a focus on Social Determinants of Health, as well as conducting initial evaluations of eligibility for health coverage and financial assistance. Additionally, the Community Health Access Manager ensures that pregnant patients and their families are connected to appropriate care and resources to support a healthy pregnancy and postpartum experience.


The Community Health Access Manager supports their team through clear accountability, recognition of achievements, and ongoing professional development opportunities, fostering a culture of excellence in patient service for all Access patients and their families. Patient Services roles are critical in ensuring patients receive and are connected to comprehensive, whole-person care that contributes to improved health outcomes.


This position collaborates with a wide range of internal stakeholders and external partners to enhance the patient experience, drive operational efficiency and continuous improvement, and ensure effective fiscal stewardship.



Required Qualifications:

  • A minimum of a high school diploma or equivalent or associateโ€™s degree in health care, social services, or related field
  • A minimum of 3 years of experience working in a healthcare, social services, or human services setting or equivalent
  • A minimum of 2 years of leadership or supervisory experience or a minimum of 5 years of Access experience
  • Proficiency in Microsoft Office Suite applications


Desired Qualifications:

  • Bachelorโ€™s degree in healthcare, human services, or a related field, or an equivalent combination of education and experience
  • Demonstrated experience with process improvement methodologies and practices
  • Knowledge of community resources and support services
  • Proven ability to build and maintain effective relationships with internal and external stakeholders
  • Experience with EPIC HealthLink preferred
