Job description

We are seeking a compassionate, skilled, and dependable Registered Nurse (RN) to join our team in an on-call capacity, working a rotating 7 days on / 7 days off schedule. This role is ideal for a nurse who excels in autonomy, critical thinking, and providing high-quality, patient-centered care during off-hours and urgent situations.
As the on-call RN, you will be responsible for responding to patient needs after hours, including symptom management, triaging urgent calls, completing admissions, and conducting death visits. You will be a critical part of our care team, ensuring continuity of care and support for patients and families during evenings, nights, weekends, and holidays when needed.
Key Responsibilities:
  • Provide timely and appropriate clinical responses to after-hours calls and visits.
  • Perform symptom assessment and management for patients experiencing urgent needs.
  • Triage patient and family concerns, determining the appropriate course of action.
  • Conduct new patient admissions as needed.
  • Respond to and manage patient deaths, ensuring compassionate care and proper documentation.
  • Collaborate with interdisciplinary team members to ensure high-quality care and communication.
  • Accurately document all assessments, interventions, and communications in the EMR.
