Job description

Southeast Healthcare is seeking an HIV Medical Case Manager to provide comprehensive case management services to individuals living with HIV, ensuring they receive the care, support, and resources they need to manage their health and well-being effectively.

Responsibilities:

  • Assess client needs through comprehensive intake interviews, medical history, and assessment of service needs.
  • Develop and implement individualized service plans in collaboration with clients.
  • Coordinate access to medical care, medications, and community resources, including housing, mental health services, and substance abuse treatment.
  • Advocate for clients within the healthcare system and help them navigate insurance and eligibility requirements.
  • Provide ongoing support, education, and counseling to clients about HIV, treatment adherence, and healthy living.
  • Monitor and track client progress, adjusting service plans as needed to meet evolving needs.
  • Maintain accurate and timely case documentation.
  • Ensure clients are connected to appropriate social services, including financial assistance for medical and dental needs, housing, and other community resources.
  • Participate in interdisciplinary team meetings to review client progress and identify additional areas of support.
  • Stay current with HIV-related research, treatment advancements, and community resources.
  • Bachelor’s & master's degree required.
  • LSW required. 
  • Experience in case management, preferably with individuals living with HIV/AIDS.
  • Knowledge of HIV care, treatment, and available community resources.
  • Strong interpersonal, communication, and advocacy skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Excellent organizational and time-management skills.
  • Sensitivity to the unique challenges faced by individuals living with HIV and marginalized populations.
  • Ability and prior experience working with computer systems necessary to maintain services for clients, reporting, funding, and other program related needs.
