Registered Nurse (RN) / RN Case Manager

We are currently seeking a dedicated and experienced Registered Nurse (RN) / Case Manager to join our team.

Position Overview

The RN Case Manager is responsible for coordinating and overseeing patient care in the home health setting, ensuring compliance with state and federal regulations while delivering high-quality clinical services.

Qualifications

  • Graduate of an accredited school of Nursing
  • Current Registered Nurse (RN) license in the state(s) of service
  • Minimum of two (2) years of experience in home health care
  • Strong clinical assessment and care coordination skills
  • Knowledge of state and federal home health regulations
  • Excellent communication and documentation skills

Key Responsibilities

Patient Assessment & Care Planning

  • Complete initial patient assessment within 72 hours of admission
  • Conduct reassessments every 60 days or as patient's condition requires
  • Evaluate:
    • Patient needs, resources, and environment
    • Care goals and treatment plans
    • Medications, including side effects and contraindications
    • Required medical supplies and durable medical equipment

Care Coordination & Case Management

  • Develop, implement, and manage the plan of care
  • Coordinate services with physicians, therapists, and care team members
  • Notify physicians of any changes in patient condition
  • Provide regular patient status updates (at least every 62 days or as needed)
  • Assist with patient transfers, discharges, and care transitions

Supervision & Team Leadership

  • Assign and supervise LPNs and home health aides
  • Conduct supervisory visits:
    • Every 2 weeks (when skilled services are provided)
    • Every 60 days (when aide-only services are provided)
  • Evaluate staff performance, competency, and quality of care
  • Ensure aides follow assigned duties and report changes in patient condition

Clinical Documentation & Compliance

  • Ensure all patient care is accurately documented in the medical record
  • Record and process physician verbal orders
  • Maintain compliance with state and federal regulations
  • Ensure therapy and professional services follow physician orders and care plans

Collaboration & Support

  • Assist physicians in evaluating patient needs
  • Participate in developing and revising care plans and goals
  • Coordinate with interdisciplinary team members
  • Support quality improvement and patient outcomes
