Job description

Spiritual Care Coordinator

The Spiritual Care Coordinator provides spiritual support and counseling to patients, families, and staff. They act as a liaison between patient/client/family and local congregations and the Hospice Program to identify and meet spiritual needs. They coordinate bereavement activities and services and collaborate with members of the patient care team in identifying and resolving spiritual and emotional problems. These responsibilities are carried out in accordance with organization policies/procedures and standards consistent with Federal, State and accreditation regulations, as well as Organization and other regulatory organizational requirements.

Duties & Responsibilities
  • Provides spiritual support and counseling to patient and family members in the home and in-patient settings as needed as well as for agency staff.
  • Provides assessments of patient/client family spiritual/bereavement care needs.
  • Assists Hospice Bereavement staff in consultation with Program Manager to hospice clients/families and community (support groups).
  • Acts as a liaison with patient/client/family clergy and congregation as requested during patient illness and during bereavement.
  • Offers assistance with funeral/ memorial services.
  • Documents visits to and interventions with patients/clients and families.
  • Updates team on patient/family needs via hospice line.
  • Completes bereavement risk assessment from admission through death.
  • Assists in preparations for the annual Hospice Interfaith Memorial Service and other community outreach activities as requested, as well as Grief Support Groups in community as needed.
  • Actively participates in Hospice Interdisciplinary Team meeting, identifying needs and problems, interventions, outcomes and documents on IDT forms.
  • Participates as instructor for volunteer training sessions and hospice in-service as requested.
  • Participates in continuous quality improvement activities.
  • Serves on appropriate agency committees as needed.
Requirements
  • Bachelor's Degree Required; Field of Study to include completion of curriculum of theological classes. Master of Divinity (M.Div.) or Master of Theological Studies (MTS) preferred.
  • Minimum 1 to 3 Years of spiritual care experience required with emphasis on providing spiritual/bereavement support for Hospice/Healthcare patients and their families.
  • Experience with grief support beneficial.
  • Must possess valid driver's license and current auto liability insurance.
  • Minimum of two units of clinical pastoral education preferred through the Association for Clinical Pastoral Education.
  • Displays skills and competencies in spiritual care.
  • Initiative and good judgment.
  • Excellent communication and organizational skills.
  • Must be able to work independently and complete assignments with minimal supervision.
  • Must be able to coordinate, prioritize and complete tasks simultaneously in a timely manner.
  • Must be customer service oriented and able to blend strong clinical knowledge with excellent problem-solving and organizational skills.
  • Excellent interpersonal skills required.
  • Leadership skills and the ability to continuously motivate others.
  • Knowledge of computer systems preferred.
  • Must be able to speak, read, and write English with excellent oral and written communication skills required.
