Salary: $45 - $60 Per-Diem

We are looking forLicensed Vocational Nurse (LVN)with a background inhome health care experience. Applicants must havepriorexperience in home health nursing to be considered for this role.


Our agencyEvolve Home Healthis located in Agoura Hills, CA, we provide services throughout Los Angeles County and extending into the Mid/Central Valley. (Fresno, Clovis, Madera, Coalinga, Mendota, Merced, Tulare, Visalia, San Diego area, and Los Angeles County)


ESSENTIAL FUNCTIONS/SKILLS

  • Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
  • Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
  • Document all services provided and submit them on appropriate forms.
  • Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
  • Report changes in patient condition and needs to RN
  • Great time management skills
  • Performs wound care and Wound Vac
  • Dementia care
  • Tracheostomy care


VISIT TYPES

  • Routine/Follow-Up visits: monitoring chronic conditions
  • Take vital signs, weight, and basic assessments
  • Wound care under standing orders
  • Skilled nursing visits under RN Delegation: Administering medications; Trache care; ostomy care
  • Dressing changes, wound care maintenance
  • Post-Discharge or Resumption of Care Visits
  • Checking on patient progress and reporting back to the RN or DCPS
  • Patient Education/Support


QUALIFICATIONS

  • Current Licensed Vocational Nurse (LVN) license in the state of California
  • Proficiency in using electronic health record systems (e.g., DataSoftLogic)
  • Familiarity with durable medical equipment (DME) and its use in patient care
  • Minimum of 2-3 years as a Licensed Vocational Nurse (LVN)
  • Valid Drivers License, reliable mode of transportation, and proof of insurance
  • BLS/CPR/ACLS Card
  • Previous Hospice experience required


OTHER REQUIREMENTS

  • Willing to travel up to 100% of the time for business purposes
  • Flexibility to travel within assigned serviced areas
