Nurse Care Coordinator (RN)

Clarity Pediatrics is reimagining pediatric chronic care, starting with ADHD. After successfully launching our virtual clinic for ADHD and anxiety, we are expanding our pediatric care platform to address other common chronic conditions and are focused right now on Obesity and Overweight management. Our clinical model is tech-enabled, backed by the best science, and designed to deliver exceptional health outcomes that transform a child's trajectory.

Our co-founders and leadership team have deep commitment to clinical excellence and domain expertise in healthcare. Our CEO, Christina LaMontagne, has led two startups and earned her MBA at Harvard Business School. Our Chief Medical Officer, Alesandro Larrazabal, MD, trained at UCSF and Stanford. Until recently, he was a Medical Director building specialty care services at Kaiser Permanente. Earlier, he worked at IDEO.

The Nurse Care Coordinator (RN) is an integral member of Clarity Pediatrics' interdisciplinary care team, collaborating with physicians, advanced practice providers, psychologists, and therapists. Under the direction of clinical leadership, the Nurse Care Coordinator ensures seamless care delivery for children with chronic conditions by coordinating clinical appointments, providing patient and family education, managing medication authorizations, and serving as a primary point of contact between families, providers, and pharmacies. Note this is a fully remote position.

Care Coordination & Scheduling

  • Coordinate, schedule, and reschedule clinical appointments across the care team
  • Manage referrals to specialist and external clinical services
  • Proactively follow up with families to reduce gaps in care and no-show rates
  • Support care transitions and continuity across the interdisciplinary team

Communication

  • Provide clinical updates to referring physicians
  • Coordinate lab work and documentation requirements for treatment planning with referring providers
  • Support the providers for patients requiring escalation of care

Patient & Family Education

  • Provide evidence-based education to families regarding chronic condition management
  • Explain medication regimens, treatment plans, and care goals in accessible language
  • Respond to patient and family inquiries via messaging and phone in a timely manner
  • Support health literacy and empower families to engage actively in their child's care

Medication Management & Pharmacy Coordination

  • Initiate and manage medication prior authorization requests with insurance payers
  • Communicate with pharmacies to resolve dispensing issues and coordinate refills
  • Track authorization status and escalate delays to the appropriate clinical team member
  • Maintain accurate medication records and document all pharmacy communications

Documentation & Team Collaboration

  • Document clinical interactions accurately and in a timely manner across multiple EHR and care management platforms
  • Participate in interdisciplinary case conferences, team huddles, and quality improvement meetings
  • Report patient progress and care gaps to clinical supervisors
  • Identify workflow inefficiencies and contribute to process improvement solutions

Qualifications

  • Licensure: Active California Registered Nurse (RN) license in good standing (Associate's, Bachelor's, or Master's degree in Nursing accepted)
  • Tech-Savvy: Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systems
  • Communication: Strong organizational, communication, and interpersonal skills
  • Virtual Environment: Ability to manage competing priorities and work collaboratively in a remote, virtual environment
  • Cultural Responsiveness: Comfort working with diverse, medically and socially complex pediatric populations

  • Internet Access: Reliable internet connection and a dedicated workspace suitable for remote work

Logistics

  • Full-time, contract position (40 hours per week to start)
  • Due to our clinic hours, this position will be M - F from 12 PM - 9 PM PST with a 1 hour break daily
  • Occasional Saturday coverage (to be decided in advance)
  • $70,000- $90,000/year with medical and ancillary benefits (depending on Geographic Location)
