What You'll Do:
Conduct wellness assessments and monitor resident health changes
Support medication management and perform MAR/TAR audits
Communicate with families, physicians and care teams
Monitor incidents, falls and follow-up care
Maintain documentation and compliance with regulations
Support infection control and clinical quality initiatives
Train and support care team members
What You'll Bring:
Active LPN/LVN license
1+ year experience in senior living or clinical setting
Strong assessment and documentation skills
Knowledge of medication administration and compliance
Strong communication and problem-solving skills
