Responsibilities:
  • Provide direct patient care including assessments, treatment planning, and administering medications. Monitor and document patient progress and respond to changes in condition.
  • Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans for residents.
  • Effectively communicate with residents, families, and healthcare team members to address concerns and provide updates on patient status.
