Job Title: Weekend Hospice On Call Registered Nurse

Territory: Entire service area 


Job Summary: Primary functions are to administer nursing care for patients of all ages in their place of residence after hours and/or on weekends. The On Call Registered Nurse will be responsible for responding to urgent patient and family needs, managing symptom control, and providing support during end-of-life care. This role requires a timely response to calls, effective clinical assessment, and the ability to work independently.
Job Qualifications:

Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.

Licensure: Current state license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver's license.

Experience: Experience as a registered nurse in a clinical care setting. Hospice experience preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write, and comprehend English.

Transportation: Reliable transportation and valid auto liability insurance.


Environmental and Working Conditions:Works in patients' homes and facilities in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.


Essential Functions:

  • Provide on-call hospice nursing care in response to patient or family needs during assigned shift.
  • Perform patient assessments, manage symptoms, and administer medications as needed.
  • Collaborate with the interdisciplinary team (physicians, social workers, chaplains, hospice aides, etc.) to ensure comprehensive care.
  • Consults with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Maintain accurate and timely documentation of care provided using EMR.
  • Communicate changes in patient condition to the care team and recommend adjustments to the care plan.
  • Provide emotional support and guidance to patients, families, and caregivers.
  • Ensure that all interventions adhere to hospice philosophy, polices, and regulatory guidelines.
  • Maintain timely response to on-call duties, ensuring patient and family needs are met in a timely manner.
  • Triage duties may be required for some employees in this role, depending on specific assignments and departmental needs.
  • Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Informs physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient and family response to care.
  • Demonstrates commitment and professional growth and competency by participating in the Hospice sponsored in-service programs and maintaining or improving competency.
  • Promotes the Hospice's philosophy and administrative policies.
  • Provides effective communication to patients, family, team members, and other health care professionals.
  • Perform emergency admissions of new hospice patients as needed.
