Registered Nurse – Weekend Hospice RN (After-Hours)

Location: Milwaukee North Market (Washington, Ozaukee, Dodge, Waukesha, Milwaukee Counties) Schedule: Every Saturday & Sunday, 8:00 AM – 8:00 PM Type: Contract | 24 hours/week | No Caseload

Job Summary

This role is all about stepping in when patients and families need support the most. The Weekend Hospice RN provides after-hours hospice visits, end-of-life care, PRN symptom management, and death visits throughout the Milwaukee North region. Unlike traditional case-manager roles, this position does not carry a caseload —you focus strictly on same-day needs, delivering compassion, guidance, and clinical expertise wherever it's needed.

Key Responsibilities Patient Care & Clinical Duties
  • Provide in-home hospice nursing care, including symptom management, comfort measures, and end-of-life support.
  • Respond to same-day PRN requests such as pain crises, increased symptoms, respiratory distress, or urgent family concerns.
  • Perform daily scheduled visits for patients nearing end of life.
  • Complete death pronouncement visits per state/facility guidelines.
  • Assess patient conditions, update care plans, and communicate changes to the interdisciplinary hospice team.
  • Provide emotional support, education, and reassurance to patients and families during critical moments.
Communication & Collaboration
  • Coordinate care updates with on-call leadership, hospice team members, and medical directors as needed.
  • Document all patient encounters accurately and promptly in accordance with agency standards.
  • Communicate clearly with families about the patient's condition, expected changes, and comfort-focused interventions.
Compliance & Safety
  • Follow hospice regulatory standards, Facility's policies, and state nursing requirements.
  • Maintain accurate, compliant, and timely charting of all patient interactions.
  • Ensure safe care delivery during home visits, including assessing the environment for risks.
Qualifications
  • Current RN license (State of Wisconsin – or compact).
  • Minimum 1 year of hospice, palliative care, or home health experience strongly preferred.
  • Valid driver's license and reliable transportation to travel within the Milwaukee North Market.
  • Strong assessment skills and the ability to work independently in the field.
  • Compassionate communication style with patients and families facing end-of-life situations.

  • No on-call requirements beyond the 8a–8p weekend shift.
