Location: Toledo, OH

This position is Monday-Friday 8:00am-5:00pm with no weekend or on call requirements.
As a Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare amp; Hospice, you will play a key role in an organization with a reputation for unsurpassed quality care, a close-knit, family-friendly culture, and a supportive management team as we begin to introduce hospice services to our Toledo, OH territory.
We will rely on you to assess, plan and implement patient care; educate patients and families on comfort care processes, procedures and medications; and address various patient needs.
