What You Will Do as a Hospice RN Field Nurse

As a Hospice RN Field Nurse, you will provide support to patients and families to ensure a safe and comfortable environment.
Some of your primary responsibilities will include:
  • Completing an initial assessment of patient and family to determine hospice care needs. Providing a complete physical assessment and history of current and previous illness
  • Developing a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative actions
  • Re-evaluating and revising patient care needs as needed
  • Administering medications and treatment safely and as prescribed by the physician
  • Counseling and educating patients and families in meeting nursing and related needs
  • Providing health care instructions to the patient as appropriate per assessment and plan of care
  • Participating in on-calls

Education, Licensure & Certification Requirements
  • Graduate of an accredited school of nursing
  • Active RN license
  • One (1) to two (2) years of recent acute care experience
  • Bachelor's degree with one (1) year of hospice/home health experience preferred
