A&C Private Home Care has been a trusted provider of home health services to medically fragile adults and children since 2015. Located in Norcross, Georgia, we are a growing home care agency that takes a client-centered approach to meeting the needs of those we serve-one client at a time. Our dedicated team includes compassionate caregivers, Care Coordinators, and professional clinical nurses who work together to provide exceptional care and support.

Position Overview

A&C Private Home Care is seeking an experienced, motivated, and compassionate Home Healthcare Care Coordinator to join our growing team. The ideal candidate will have at least two (2) years of experience in home care coordination, staffing, scheduling, or a related healthcare field and possess strong communication, leadership, organizational, and problem-solving skills.

This role is critical to ensuring quality client care, caregiver support, operational efficiency, and regulatory compliance. The successful candidate will interact daily with clients, client representatives, caregivers, case managers, and referral sources while managing multiple priorities in a fast-paced environment.

This position also offers an opportunity for professional growth and leadership, including the potential to supervise, mentor, and support Staffing Coordinators as the agency continues to expand.

Key Responsibilities
  • Recruit, screen, interview, hire, and onboard caregivers

  • Schedule caregivers with clients and manage ongoing staffing needs

  • Coordinate and document client visits and caregiver assignments

  • Conduct routine client wellness and satisfaction check-ins

  • Address and resolve concerns reported by clients, caregivers, case managers, and referral sources

  • Ensure caregivers understand and follow care plans, company policies, and procedures

  • Maintain accurate documentation and communication records

  • Collaborate with clinical staff and management to ensure quality service delivery

  • Participate in the on-call rotation, including weekends, with a designated weekday off

  • Provide direct care services when emergency coverage is needed

  • Assist with compliance monitoring and caregiver performance management

  • Help train, mentor, and support Staffing Coordinators

  • Assist management with workflow oversight, quality assurance, and process improvement initiatives

  • Monitor open shifts and staffing needs to ensure timely coverage and client satisfaction

Qualifications
  • Minimum of two (2) years of experience in home healthcare coordination, staffing, scheduling, case management support, or a related healthcare field

  • Previous caregiving experience required

  • Supervisory, training, mentoring, or leadership experience preferred

  • Excellent verbal and written communication skills

  • Strong organizational, multitasking, and time-management abilities

  • Ability to work effectively in a fast-paced environment while maintaining attention to detail

  • Professional, dependable, and team-oriented attitude

  • Strong conflict-resolution and problem-solving skills

  • Proficiency in Microsoft Office and general computer applications

  • Current CPR/First Aid Certification

  • Current TB Skin Test or TB Screening

  • Successful completion of a fingerprint background check through FieldPrint
