Our client, Beyond Disabilities is seeking a detail-oriented Care Coordinator to support our waiver services program. This role is responsible for coordinating Individual Service Plans (ISPs), ensuring documentation compliance, and serving as a key liaison between families, Support Coordinators, and Direct Support Professionals.

Job Description

Case Coordination & ISP Management

  • Coordinate Individual Service Plan (ISP) meetings with Support Coordinators and families
  • Ensure ISP Parts I–V are accurate and certified for each individual served
  • Complete and manage ISAR submissions in WAMS
  • Address pended or rejected authorizations and follow up with Support Coordinators
  • Conduct case reviews to ensure compliance with service plans

Documentation & Compliance

  • Review ISP documentation in WAMS and Therap systems
  • Verify quarterly reports for accuracy and compliance before final approval
  • Monitor documentation to ensure alignment with DMAS and DBHDS regulations
  • Provide documentation guidance and training to Direct Support Professionals

Service Coordination

  • Serve as a primary contact for Support Coordinators and families
  • Follow up on service authorizations, discharges, and new referrals
  • Maintain clear communication with internal teams and external providers

Operational Support

  • Conduct monthly onsite visits for individuals receiving services
  • Review individual records for compliance
  • Monitor staff documentation and service delivery accuracy
  • Support leadership with compliance reports and recommendations
Qualifications

Preferred candidates will have:

  • Experience in developmental disability services, waiver services, or case management
  • Knowledge of Individual Service Plans (ISPs) and documentation requirements
  • Experience with WAMS, Therap, or similar documentation systems
  • QMHP, QDDP or QIDP Certified
  • Strong organizational and communication skills
  • Ability to manage multiple cases and deadlines is a Must
  • Bachelor's degree in health/human services preferred
  • Commitment to confidentiality and professional standards
