What you will be doing:

The Virtual Care Nurse will be working in close collaboration with a diverse and high functioning patient care delivery team, the Virtual Care Nurse is a central resource in caring for patients remotely. The overreaching goal of the Virtual Care Nurse is being responsible and accountable for coordinating timely, safe, effective care to remote patients using a high level of technical and organizational skills as well as critical thinking ability.

The Virtual Care Nurse will be responsible for the coordination of all remote patient care activities, including screening, triage, coordination of a multi-disciplinary team for safe, timely and high-quality care. This responsibility requires the Virtual Care Nurse to be a proficient and experienced clinician possessing an in-depth knowledge in all aspects of nursing care. It also requires frequent collaboration with customers, physicians, and leaders, requiring expert level communication skills.

  • Use monitoring, assessment, planning, intervention, and critical thinking skills to care for acute care patients in a virtual/remote patient care program. Observes, evaluates and expeditiously alters plan of care based on signs and symptoms of illness, reactions to medications/treatments, general behavior, and/or general physical condition.
  • Demonstrates advanced assessment skills, integrating appropriate data sources to assess the signs, symptoms, reactions, behaviors, or general appearance to determine normal versus abnormal characteristics. Uses nursing knowledge to identify, solve, and prevent complex patient problems.
  • Conducts virtual patient care visits for diverse patient populations as directed by the care team and protocol. Plans and implements individualized patient care in collaboration with interdisciplinary teams based on observations. Implements appropriate reporting, referrals and care in accordance with standardized procedures. Initiates emergency procedures when indicated.
  • Leverages the resources and expertise of the virtual care team and service provider network to implement collaborative workflows, policies, protocols, and guidelines that support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation
  • Delegates activities or tasks related to patient care while retaining accountability for the outcome and responsibility for the delivery of nursing care
  • Actively facilitates new patient admissions; assumes additional patient intake responsibilities as directed by program leadership; implements established guidelines throughout intake process
  • Provides and precisely documents nursing process components and relevant interventions for direct and indirect patient care services that ensures the safety, comfort, personal hygiene, and protection of patients in a timely manner.
Educational/Certification Requirements:
  • Associates Degree in Nursing (ADN) is required.
  • Bachelor's Degree in Nursing (BSN) preferred.
Licenses/Certifications Requirements: 
  • California RN License is required.
  • Advanced Cardiovascular Life Support (ACLS/ALS) is required.
  • Basic Life Support (BLS) is required.
  • ANCC Specialty Certificate is required for internal candidates. (New hires may be given a year to obtain)
Experience Requirements:
  • Five (5) years of acute care nursing experience in a specialty area is required.
  • Three (3) years of acute care nursing experience in critical care is preferred.
  • One (1) year of Charge or Supervisory experience is preferred.
  • One (1) year of Clinical Nurse III experience for CSMC employees is preferred.
  • One (1) year of Virtual Care Nurse and/or nursing informatics experience is preferred.
