Job description

Physical Therapist

Job Title: Physical Therapist

Profession: Therapy/Rehabilitation Specialty: Home Health

Duration: 13 weeks

Shift: Day

Hours per Shift: 8 hours (08:00 - 17:00)

Experience: Minimum 1 full year of Adult Home Health Experience

License: Must have a license in hand

Certifications: CPR, BLS

Description: Home visits for therapy services. Periodic office meetings required. Must have reliable transportation, current driver's license, and agency-required liability insurance. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. Visits and activities hold different point values. Contractor must hit weekly productivity points of 30. Types of cases include Start of Care and Follow Ups. Weekend requirements vary per location and profession. Must be willing to travel up to 75 miles from home. Mileage will be reimbursed at the IRS standard rate. Local candidates are accepted. No pending licenses will be accepted. All required documentation must be completed within 24 hours of the appointment. Expectations include accepting every patient on the schedule regardless of distance. Communication about low patient census and financial needs is encouraged. No guaranteed hours are associated with this contract. Being open to traveling to multiple locations is essential. Compliance with specific electronic medical record systems is expected. Candidates must have experience in home health.
