MISSION STATEMENT:
The Divisions of Vascular Surgery & Otolaryngology-Head and Neck Surgery at Rutgers NJMS is the regional leader in providing quality clinical care to patients with vascular and otolaryngologic diseases, educating future vascular surgeons and otolaryngologists, and advancing care through clinical and translational research,
SUMMARY:
APN will be an integral member of the Vascular Surgery & Otolaryngology Teams at Rutgers NJMS, providing care to patients, improving efficiency of care, and participating in both quality improvement and clinical research programs,
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following,

Communicates clearly to ensure understanding
Responds to complaints and concerns by resolving issues immediately whenever possible
Interacts with all members of the multidisciplinary team to ensure the plan of care is coordinated
Meets or exceeds patient/family expectations for service
Acts as a role model for Service Excellence
Maintains professional composure and confidence in the care setting
Works effectively with team to ensure customer service scores for work area meet/exceed targets
Demonstrates respect for patient rights and is committed to the uniqueness of the individual patient,
Collects comprehensive patient health data/history including past and present medical, surgical, family and social history
Diagnostic tests and procedures relevant to the patient s status are collected, interpreted and documented at the Electronic Medical Record
Completes the medication reconciliation process by documenting the current list of medications taken by patient at each visit
Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management
Evaluates patients in the Emergency Department and for inpatient consultations and assists in management of these patients
In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis
In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes,
Evaluates laboratory studies and diagnostic tests and provides appropriate interventions as required,
Evaluates patient progress in attaining expected treatment outcomes
Monitor for signs and symptoms related to disease progression/recurrence
Appropriately manages medical and anticoagulation management emergencies according to protocol/policy
Prescribes and/or orders medications
Obtains appropriate consents for surgery and procedures
Ensures completion of all needed pre-operative tests, forms and documents,
Document assessments, prescribing, plan, modifications to plan, and evaluation of care in the electronic medical record
Triages patients problems via telephone calls
Provides monitoring and continuity of care between departments/services; collaborates with Clinical Care Coordinators, Patient Navigators and medical staff to coordinate continuity of care
Collects relevant data and prepares patient s discharge packet, included needed prescriptions and follow-up office practice visits
Performs daily rounds, independently and with physicians and performs needed clinical tasks including progress notes, orders, dressing changes, and other care as needed
May insert and remove central venous catheters, drains and arterial lines
May remove arterial sheaths
May provide tracheostomy and gastrostomy care
May perform flexible laryngoscopy and nasal endoscopy, and Incision and Drainage
Assist during outpatient/bedside surgical procedures,
Coordinists for patient care
Dictates discharge summaries, dictates comprehensive discharge and/or interim update letter to referring physicians
Assists in bed management including: Arranges patient admission and transfer, Arranges patient transfer between all levels of care
Interfaces with families to obtain and provide information concerning patient s medical care and progress
Monitors intended and potential side effects of medications and treatments ordered and will offer appropriate, accepted interventions
Provides health promotion and disease/illness/injury prevention
Provides patient and family counseling and reassurance in response to psychological or psychosocial issues
Educates patients, families and caregivers:
disease etiology to facilitate adherence to treatment, supportive care and follow-up expected and potential adverse effects
costs of prescribed pharmacologic and non-pharmacological treatments and interventions
risks and benefits of alternative, integrative and complementary therapies
Participates in informal instruction of nursing students and other health professionals
Provides pre and post-operative patient education with particular emphasis on the PAT and post recovery process to ensure patient and family compliance and satisfaction with their healthcare experience to meet the needs of the patient/family
Oversees patient pre-and post-op teaching for discharge planning,
Participates in the Division s Vascular Surgery teaching conferences
Interact with Vascular Surgery and General Surgery residents for efficient and compassionate patient care
Assist the divisions with the development of patient education programs; ie Smoking cessation, cholesterol and lipid management, tracheostomy care
Completes forms used to prepare bills and claims, if appropriate
Participates in performance improvement activities and implements changes as identified
Joint Protocols are reviewed through an annual Collaborative Practice Agreement with the Collaborating Physicians and signed both parties
Understands and adheres to Rutgers compliance standards as they appear in RBHS s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy
Performs other related duties as assigned
EDUCATION AND/OR EXPERIENCE:
Credential Required: Current NJ License and Registration; Certified as an Advanced Practice Nurse, by the Board of Nursing, with specialization as indicated or Physician Assistant through the New Jersey State Board of Medical Examiners, BLS, CDS and DEA Licenses
A current Registered/Professional Nurse license and registration in the State of New Jersey is required
Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master s degree in nursing and certified by a national certifying body
Evidence of excellent interpersonal skills, effective communication skills, and creative problem solving are essential
A minimum of two years of clinical nursing experience and current BLS certification are required,
