Unity Support Services is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. • Conduct thorough assessments of clients' social, emotional, and environmental needs to develop a nuanced understanding of their situations.
• Create individualized service plans that address specific goals, interventions, and methods for monitoring progress.
• Provide empathetic counseling and therapeutic support to clients and their families as they navigate the emotional complexities of hospice care.
• Work collaboratively with clients to establish personalized objectives, integrating their wishes and needs into service delivery.
• Implement crisis intervention strategies and perform risk assessments when necessary, ensuring client safety and well-being.
• Engage with community organizations and stakeholders to coordinate services and advocate for the essential needs of your clients.
• Conduct outreach activities to promote the accessibility of hospice services and raise awareness about available resources.
• Participate actively in case conferences and team meetings to ensure comprehensive care and continuous improvement in service delivery.
• Maintain detailed documentation and records that comply with HIPAA regulations and organizational policies.

Your work will primarily focus on terminally ill patients and their families who are coping with grief, loss, and emotional distress. Understanding the sensitivities surrounding hospice care, you will play a crucial role in providing psychosocial support, helping clients articulate their needs, and accessing essential resources. Every interaction will aim to validate their experiences and support them through a dignified end-of-life process.

• A Master's degree in Social Work from an accredited institution is required.
• An active license as a Licensed Clinical Social Worker (LCSW) in the state of California is essential.
• Demonstrated understanding of hospice care principles and experience working in related settings.
• Comprehensive knowledge of trauma-informed care, along with an understanding of social and economic justice issues.
• Exceptional communication, cultural competence, and problem-solving capabilities.

• Prior experience in providing counseling services within a hospice or palliative care setting is highly desired.
• Familiarity with local community resources that support terminally ill patients and their families.
• Proficiency in managing case documentation and utilizing electronic health records (EHR) systems.
• Experience in developing and leading workshops or support groups for clients and families is a plus.

The annual salary for this position ranges from USD 60,000 to 100,000, commensurate with experience and qualifications. In addition to competitive compensation, we offer a robust benefits package:
