Key Responsibilities
  • Conduct thorough assessments of clients' social, emotional, and environmental needs to develop a nuanced understanding of their situations.
  • Create individualized service plans that address specific goals, interventions, and methods for monitoring progress.
  • Provide empathetic counseling and therapeutic support to clients and their families as they navigate the emotional complexities of hospice care.
  • Work collaboratively with clients to establish personalized objectives, integrating their wishes and needs into service delivery.
  • Implement crisis intervention strategies and perform risk assessments when necessary, ensuring client safety and well-being.
