Job description

AS A WEEKEND TRIAGE HOME HEALTH REGISTERED NURSE (RN), you will experience a rewarding career by providing in-home nursing services for our home health patients through evaluating and assessing patients and coordinating all Home Care services in the patient's home.
$20,000 Sign-On/Retention Bonus for Qualified Candidates
Multiple Service Locations Available
  • Fleet vehicles available for eligible employees
    • 2026 Subaru Crosstrek
    • Fleet Vehicles are permitted to be used for work AND personal use
    • All fuel costs for a fleet vehicle (for work AND personal) are covered by PHH
Schedule: You'll work 10 hour shifts every Friday, Saturday, Sunday. You'll be on-call from Friday at 4:30pm until Monday at 8:00AM.
QUALIFICATIONS:
  • Have a current Pennsylvania RN License
  • Have a valid PA driver's license and reliable transportation
  • Have current Automobile coverage
  • Have 2 years of acute care or long-term care experience
  • Have excellent time management skills
  • Have strong verbal and written communication skills
  • Have strong teaching skills
  • Have ability to work independently and within a multidisciplinary team
  • Have ability to meet physical requirements of position
