Job description

Job Purpose:
- The Triage Nurse at Wildwood Family Clinic is responsible for assessing patient needs over the phone or in person, determining the urgency and type of care required, and providing appropriate guidance or referrals. This role is essential in ensuring that patients receive timely and effective care, while also optimizing clinic resources and maintaining a high standard of patient satisfaction.
Key Responsibilities:
- Conduct initial patient assessments through phone or face-to-face interactions to determine the level of care needed.
- Prioritize patient cases based on the severity of symptoms and medical history.
- Provide clear instructions and guidance to patients regarding self-care, medication, or further medical evaluation.
- Collaborate with physicians, nurse practitioners, and other healthcare professionals to ensure seamless patient care.
- Maintain accurate and detailed patient records, documenting all interactions and care recommendations.
- Educate patients and families on health management and preventive care strategies.
- Participate in ongoing training and professional development to stay current with medical protocols and best practices.
- Adhere to all clinic policies and procedures, ensuring compliance with healthcare regulations and standards.

- Our nurses provide telephone triage and health advice to patients of all ages. Calls received can vary in subject matter and complexity for this busy family practice, with OB. In addition to triage calls, RNs will coordinate same-day appointments, provide out-patient injections, and assist providers as needed. Registered Nurses must be attentive and engaged listeners who have strong critical thinking and clinical assessment abilities to make decisions independently and to document clear clinical data.

- Priority will be given to those who have phone triage experience and/or knowledge of Epic

- Pay is commensurate with experience.

Required Education:
