What is a Triage RN Home?

Triage RN Home nurses are registered nurses who assess and prioritize patient health concerns remotely, typically via phone or digital platforms, from a home-based setting. They determine the urgency of symptoms, provide medical advice, and guide patients to the appropriate level of care, such as scheduling doctor appointments or recommending emergency care when necessary. Their role is crucial in ensuring patients receive timely, appropriate medical attention while reducing unnecessary visits to healthcare facilities.

Building a good rapport with both patients and the interdisciplinary team helps ensure effective care coordination and timely intervention.
