Job description

Good Samaritan Hospice is seeking a full time Triage Nurseย to work remotely to assist our patients in our service territory. Theย Triage Nurse is a licensed health care professional who is responsible for communicating with our patients and/or families after hours (from 4:30pm to 8am) assessing their needs in an efficient and effective manner which includes assigning a clinician to make a visit to the patient if necessary. This position would also assist with workflow duties and includes a weekend and holiday rotation since hospice is a 7 day/week service line.

QUALIFICATIONS

  • Registered Professional nurse with current valid Pennsylvania license
  • Minimum of [2] years of current experience as a professional nurse in a medical/surgical environment and [1] year home health experience preferred.
  • Skilled in critical thinking, communication, negotiation, collaboration and customer service.
