• Photo ID and facility badge required at all times

Key Responsibilities

  • Conduct psychosocial assessments for newly admitted and existing residents
  • Develop and implement social service care plans in collaboration with the interdisciplinary team
  • Provide individual and family counseling and crisis intervention as needed
  • Facilitate discharge planning including coordination of community resources and post-discharge support
  • Assist residents and families with advance directives, financial resources, and benefits navigation
  • Maintain accurate and timely documentation in the EMR per facility and regulatory standards
  • Ensure compliance with federal and state social services regulations in the LTC/SNF setting

Requirements

  • Bachelor's or Master's degree in Social Work (BSW or MSW)
  • Active state social work license (LSW, LMSW, LCSW, or equivalent) preferred
  • Minimum 1–2 years of social work experience in a LTC/SNF or healthcare setting preferred
  • Strong knowledge of discharge planning, community resources, and long-term care regulations
  • Excellent communication, documentation, and interdisciplinary collaboration skills
