Job Summary:

Elitecare Medical Staffing is seeking an experienced and compassionate Travel Social Worker to join our team in Concord, California. The ideal candidate will provide essential social work services to patients and their families, ensuring they receive the support needed for their emotional and social well-being.

Key Responsibilities:
  • Provide social work services to patients and families, addressing their emotional, social, and environmental needs.
  • Conduct assessments to determine the needs of patients and develop appropriate care plans.
  • Collaborate with healthcare teams to coordinate services and resources for patients.
  • Assist patients in navigating complex healthcare systems and accessing community resources.
  • Provide crisis intervention and support to patients facing challenging situations.
  • Document all interactions and progress in patient records in compliance with regulatory standards.
  • Participate in multidisciplinary team meetings to discuss patient care and treatment plans.
  • Maintain confidentiality and adhere to ethical guidelines in all interactions.
Qualifications:
  • Current California BBS Certification is required.
  • Basic Life Support (BLS) certification from AHA only is required.
  • Minimum of 2 years of experience as a social worker in a healthcare setting.
  • Active California licensure is required; acceptable licenses include LCSWs, ACSWs (also known as MSWs), LMFTs, and AMFTs (also known as MFTs).
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a team in a dynamic environment.
Scheduling Details:
  • This position requires an alternating schedule of 32 hours per week, rotating between Tuesday to Friday one week, and Wednesday to Saturday the next week.
