Summary:
POSITION SUMMARY:
The transplant Coordinator is responsible for the coordination of care for potential living donors. This position is a key member of the multidisciplinary transplant team that facilitates the movement of the living donation process. This role is responsible for the clinical aspects of the transplant program including but not limited to education, patient care coordination, data management, quality assessment, and community outreach and program development. Under general supervision, the Transplant Coordinator relies upon clinical skills, information sharing, as well as education, and research to promote quality care for transplant patients. Provides growth in clinical knowledge through research-based practice with peers. In addition, the transplant coordinator advances the hospital's mission by adhering to and upholding its policies and procedures.
POSITION EDUCATION/ QUALIFICATIONS:
Graduation from an accredited school of nursing as a Registered Nurse
Bachelor's Degree in Nursing required or must be attained within three years of the date of hire
Current RN licensure in the State of Texas or Compact state required
Current American Heart Association Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) Healthcare Professional certification required.
JOB KNOWLEDGE/EXPERIENCE:
• Minimum of four (4) years' nursing experience required
• Knowledge of transplant, health care trends, community and regional resources
• Ability to establish cooperative working relationships with diverse groups and individuals, medical staff and other health care disciplines is required.
• Understanding of the consultative process.
Responsibilities:
POSITION RESPONSIBILITES:
• Coordinates the care of transplant patients and manages the medical and surgical needs of patients in outpatient setting in a way that promotes optimal physical, social, and emotional rehabilitation.
• Demonstrates transplant knowledge in such areas as medication administration, complication management, and nursing assessment.
• Collaborates with all parties involved to provide the most cost-effective and appropriate care to all patients.
• Systematically ensures that all plans of care are congruent with patients' needs and in alignment with best practices.
• Ensures that the quality of the nursing care provided demonstrates compassion and a strong awareness of resource optimization.
• Provides education to transplant recipients and their families so as to clarify the demands of transplant life including pre transplant, post-transplant, and living donation. As well, the transplant coordinator provides critical information about when to call for concerning symptomatology, or to assess compliance with medication and medical regime. He /she; collaborates with transplant team to maintain compliance with medication renewal, pre-authorization protocols, and with any requirements pertaining to ad hoc changes which might affect compliance with insurance company formularies. Using current transplant protocols, the transplant coordinator conducts follow-up visits and makes decisions about the frequency of follow up care
• Assist potential donors in understanding the nature of organ donation. Performs professional nursing service in inpatient/outpatient clinical setting including, but not limited to: patient assessment, teaching, discharge planning, telephone triage.
• Advocate for the rights of the potential donor; ensure the donor's decision is informed and free of coercion. Greets patients and prepares them for physician evaluations. Screens patients for appropriate information. Instructs patients in the collection of samples and completion of tests. Facilitates the evaluation of both pre and post donation by interfacing with physicians, Transplant nephrologists, Transplant surgeons, scheduling coordinators, consultants, Insurance coordinators and social workers. Has primary responsibility for planning, implementation and evaluation of this process.
• Protect and promote the best interest of the potential donor. Maintains and reviews patient records, charts, and other pertinent information. Posts tests and examination results. Obtains and reviews consent forms, treatment plans, and side effects as appropriate. Discusses potential complications with patient and family.
• Resolve any concerns or complaints the donor candidate may have. Provide information regarding the consent process, evaluation process, surgical process and the need for continued follow up visits
• Ability to perform ("take") weekly call rotations
• Maintain current knowledge of UNOS/OPTN policies pertaining to living donation. Provide in-service for nursing staff and presentations for any outside organizations or groups.
• Participate in quality monitoring and process improvement activities and programs

• B. Working Conditions: The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
C. Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids). Person who performs these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
