Job description

Position Summary:

This person will oversee the daily operations of the Village Transitional Living including direct supervision of assigned staff. This role ensures the delivery of high-quality, trauma-informed, and client-centered services from admission through discharge, with a focus on helping clients achieve stable housing and long-term self-sufficiency.

Essential Job Functions:

  • Act as lead case manager and set standards for performance;
  • Leads weekly team meetings to ensure smooth transitions to the next level of care, community-based housing, employment and other services;
  • Oversees general client supervision and help modify interfering attitudes and behaviors precluding them from maintaining their recovery and meeting identified personal goals;
  • Maintains oversight of facility responsibilities to include record keeping (chore assignments, client schedules, facility events, apartment checks, curfew, sign in/out sheets, program expectations, client needs, regular meeting logs, etc.);
  • Uphold and ensure documentation timelines;
  • Maintain and ensure regular meetings with referring entities;
  • Provide staff training services (orientation, job duties, etc.);
  • Oversee the distribution of household items, vouchers, reimbursements, and assistance);
  • Work with the assigned team in the preparation of continuing care and discharge planning prior to discharge.
  • Coordinate with community providers to ensure that a viable network of support services are available to all clients;
  • Oversee and attends job fairs and employer events;
  • Provide daily, weekly and monthly reports as requested;
  • Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and
  • Perform any other duties as assigned.

Essential Qualifications:

Certifications/Licenses:

  • Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance is required;
  • CPR Certification, First Aid Certification and Annual Tuberculosis test
  • Ability to pass first level and NABS background clearance

During your tenure with WestCare there may be new requirements, including but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

Education:

  • High School Diploma or Equivalent is preferred

Experience and Competencies:

  • Experience consisting of two (2) years of experience as a Transitional Coordinator/Manager or related field is preferred;
  • Ability to multi-task is necessary
  • Advanced computer skills to include Microsoft Office suite
  • Excellent documentation and organizational skills
