Choice Medical Group has been serving the local area for over 30 years and has a network of over 200+ physicians. Choice Medical Group is physician owned and located in Apple Valley, CA. We are seeking a compassionate and detail-oriented Nurse Practitioner or Physician Assistant to join our care team. This role focuses on delivering high-quality preventive care through Annual Wellness Visits (AWVs), accurate HCC (Hierarchical Condition Category) recapture, and comprehensive medication reconciliation. Youll play a critical role in improving patient outcomes and supporting value-based care initiatives.

QUALIFICATIONS:
Must have valid, and kept current/in good standing throughout employment:

CA Registered Nursing License

CA Nurse Practitioner License

CA Nurse Practitioner Furnishing License

CA Physcian Assistant License

DEA

BLS

ACLS

Must not be on OIG/GSA exclusion list (qualified to see Medicare patients)

Minimum 1 year of clinical experience (HCC or primary care experience preferred)

  • Strong knowledge of Medicare Advantage, HCC coding, and risk adjustment
  • Proficient in EMR documentation and medical coding best practices
  • Excellent communication and patient engagement skills

Preferred Skills

  • CRC (Certified Risk Adjustment Coder) certification
  • Bilingual (Spanish/English)
  • Experience with in-home assessments or mobile care, willing to travel to local clinic sites within the Victorville, Apple Valley, Phelan, Hesperia.

Key Responsibilities

  • Conduct Annual Wellness Exams (AWVs) in accordance with CMS guidelines
  • Identify and accurately document chronic conditions for HCC risk adjustment
  • Perform thorough medication reconciliation and patient education
