Fairfield, California – Nephrology Nurse Practitioner Job


Clinical Role:
  • Do you know someone that might be a good fit for this position? Please ask about our generous referral fees!
  • Full-time Nurse Practitioner opportunity (Monday–Friday schedule with flexibility based on patient and nephrology partner needs)
  • Mix of in-clinic care (dialysis centers and nephrology practices) and telehealth visits
  • Provide one-on-one care for complex CKD and ESKD patients, including post-transplant populations
  • Develop care plans, deliver patient education, and guide an integrated care team
  • Coordinate care between patients, families, physicians, and multidisciplinary team members
  • Perform comprehensive health and risk assessments with strong focus on clinical documentation and coding
  • Manage longitudinal patient panels with emphasis on reducing readmissions and optimizing outcomes
  • Administer tests, screenings, and medication management as needed
  • Maintain strict confidentiality in accordance with HIPAA regulations
  • Opportunity to work both autonomously and collaboratively within a highly integrated care model

Qualifications:
  • Current California Nurse Practitioner license
  • Board Certified NP (ANCC or AANP)
  • Minimum of 2+ years of clinical experience (Primary Care, IM, Nephrology, Gerontology, or Palliative Care preferred)
  • Experience with chronic care management and medication optimization
  • DEA licensure / prescriptive authority (post-hire acceptable)
  • Active CPR/BLS certification
  • Familiarity with HCC coding
  • Valid California driver’s license

  • $10,000 sign-on bonus
  • Salary range: $150,000–$160,000 annually
