  3. School Nurses Only must ALSO PROVIDE - Photocopy of New Jersey RN License, CPR, and AED current registration, and ELECTRONIC (directly from College's Clearing House)/OFFICIAL undergraduate TRANSCRIPTS (in originally sealed envelope).

  • Mantoux Test for tuberculosis - proof of negative result.
  • New Jersey First Act Form
  • Fingerprinting (https://homeroom4.doe.state.nj.us/chr/). (previously fingerprinted substitutes must TRANSFER their fingerprints)
  • Verification of Continuous Employment completed & signed by District where prints originated (for previously fingerprinted substitutes)
